Shoulder Pain Stopping You from Living Life?

If reaching into a cupboard, putting on a jumper, lifting weights, or simply sleeping on your shoulder has become painful, you're not alone. Shoulder impingement is one of the most common shoulder conditions I see in clinic, and in most cases it responds really well to the right treatment.

What patient say

I'm Ellie, a physiotherapist with a passion for helping people get back to doing the things they enjoy without pain. Whether your shoulder pain has come on gradually, started after an injury, or is affecting your work, sport, or everyday life, my aim is to understand exactly what's causing it and create a rehabilitation plan that works for you.I don't believe in quick fixes or handing out the same exercise sheet to everyone. Every shoulder is different, and every treatment plan should be too.

What Is Shoulder Impingement?

Despite the name, shoulder impingement isn't always about something being "trapped" or "pinched."Current research tells us that shoulder pain is usually more complex than that. It often develops because the tendons, muscles, or other soft tissues around the shoulder become irritated when they're overloaded or not working as efficiently as they could. That's why I don't simply treat the painful area. During your assessment, I'll look at how your shoulder moves, how strong it is, how your shoulder blade functions, and whether there are any other factors contributing to your symptoms.Treating the underlying cause gives you the best chance of long-term recovery rather than temporary relief.

What to Expect at Your Appointment

During your impingement assessment, I look closely at how your shoulder moves โ€” especially overhead โ€” to identify where the tendons or bursa are being irritated. I assess strength, posture and movement patterns to understand whatโ€™s contributing to the โ€œpinchingโ€ sensation.Treatment may include handsโ€‘on techniques to reduce irritation and improve movement. I show you how to adjust certain activities or gym exercises to avoid aggravation, and I design a strengthening programme to help your shoulder move more freely and comfortably.Youโ€™ll leave with clear guidance on what movements to modify, what exercises to begin and how to progress safely.

Common Symptoms

You may have shoulder impingement if you experience:
Pain when reaching overhead
Pain lifting objects
Pain putting on clothing
Pain washing your hair
Difficulty sleeping on the affected side
Weakness during lifting
Sharp pain reaching behind your back
Pain during gym exercises such as shoulder press or bench press
Clicking or catching sensations
Reduced shoulder movement
Symptoms often develop gradually but can also occur after injury.

What Causes Shoulder Impingement?

Many different factors can contribute to shoulder pain.
Common causes include:
Repetitive Overhead Activities
Painting, decorating, warehouse work, manual labour and certain sports place repeated stress on the shoulder.
Poor Shoulder Strength
Weakness in the rotator cuff and shoulder blade muscles can reduce shoulder stability.
Sports Injuries
Swimming, tennis, CrossFit, rugby, cricket, volleyball and weightlifting commonly overload the shoulder.
Poor Posture
Long hours sitting at a desk can contribute to altered shoulder mechanics
.Previous Shoulder Injury
Previous dislocations, fractures or rotator cuff injuries can increase the likelihood of persistent shoulder pain.
Age-Related Changes
Natural tendon changes may make the shoulder more sensitive to increased activity.

Shoulder Impingement Exercises

Exercises are often the cornerstone of recovery.
Depending on your assessment, your programme may include:
Rotator cuff strengthening
Shoulder blade control exercises
Mobility exercises
Resistance band work
Progressive loading
Functional strengthening
Return-to-sport drills
Your programme is progressed as your shoulder improves.

How Long Does Recovery Take?

Recovery varies depending on:
Severity of symptoms
Duration of pain
Activity levels
Previous injuries
Commitment to rehabilitation
Many patients begin noticing improvements within several weeks, while longer-standing cases may require a more extended rehabilitation programme.
Consistency is the biggest predictor of success.

Can Shoulder Impingement Heal Without Surgery?

In many cases, yes.
Research shows that most people improve with structured physiotherapy and progressive exercise.
Surgery is generally only considered when:
Significant structural damage exists
Conservative treatment has failed
Symptoms remain severe despite rehabilitation
Our aim is always to help you recover using the least invasive treatment possible.

Frequently Asked Questions

Is shoulder impingement permanent?

No. Most people recover well with the right rehabilitation programme.

Should I stop exercising?

Not necessarily. Complete rest is rarely helpful. I can guide you on how to modify your activities so your shoulder can settle while you stay active.

Can I still go to the gym?

In most cases, yes. Many exercises can be adapted to reduce irritation while maintaining your overall fitness.

Does shoulder impingement show on an MRI?

Not always. Imaging findings donโ€™t always correlate with pain, which is why a thorough clinical assessment is essential.

Will massage fix shoulder impingement?

Massage may provide shortโ€‘term relief, but longerโ€‘term improvement usually comes from addressing strength, movement, and load tolerance.

Is shoulder impingement the same as a rotator cuff tear?

No. They can produce similar symptoms, but they are different conditions and require proper assessment to distinguish.

When Should You See a Physiotherapist?

Seek professional advice if:
Pain lasts more than two weeks
Shoulder pain affects your sleep
You struggle to lift your arm comfortably
Symptoms keep returning
Pain limits work, sport, or daily activities
Home exercises arenโ€™t helping
Early assessment often leads to better outcomes.
